REPOTED BY SANCHITA CHATTERJEE, KOLKATA: A groundbreaking achievement in heart valve treatment in Eastern India: Under the leadership of renowned structural heart specialist Dr. Sumanta Mukherjee, Apollo Multispecialty Hospitals, Kolkata, has successfully performed a new interventional procedure—Transcatheter Tricuspid Valve Replacement (TTVR)—for the first time in the country.

With this operation, Kolkata has reached a historic milestone in the field of structural heart intervention. For the first time in India, two consecutive complex Transcatheter Tricuspid Valve Replacement (TTVR) procedures have been successfully performed under the leadership of renowned structural heart specialist and Senior Consultant Interventional Cardiologist, Dr. Sumanta Mukherjee.

TTVR offers a life-saving, non-surgical alternative for patients suffering from severe tricuspid valve regurgitation (leakage of the valve on the right side of the heart) for whom conventional open-heart surgery poses an extremely high risk.

*Function of the Tricuspid Valve*

The tricuspid valve regulates blood flow between the right atrium and the right ventricle. When this valve fails to close properly, blood leaks backward (tricuspid regurgitation or TR), leading to symptoms such as fluid retention, organ failure, shortness of breath, fatigue, and ultimately, heart failure. Previously, the tricuspid valve was referred to as the "forgotten valve" because surgical intervention carried an extremely high mortality risk, particularly for elderly or frail patients.

*What is TTVR? How does it work?*

TTVR is a state-of-the-art, minimally invasive cardiac intervention. A bioprosthetic valve is compressed and delivered to the heart via a narrow catheter inserted through a small puncture in the femoral or jugular vein. Once correctly positioned within the damaged native valve, the new valve is expanded, immediately restoring proper blood flow.

*Key Benefits for Patients:*

- No Open-Heart Surgery: Does not require opening the chest (sternotomy) or using a heart-lung bypass machine.

- Rapid Recovery: Patients can walk within a few hours and are typically discharged from the hospital in 2 to 3 days, whereas open-heart surgery requires weeks of recovery.

- Immediate Relief: Symptoms such as swelling, shortness of breath, and heart failure subside quickly.

*Clinical Case Highlights: TTVR at Apollo Kolkata*

The efficacy of this procedure was demonstrated in two high-risk patients under the supervision of Dr. Sumanta Mukherjee and Apollo Kolkata’s dedicated structural heart team.

- High-Risk Patient Profile: Both patients were elderly and frail, suffering from severe tricuspid regurgitation; conventional open-heart surgery was deemed unsafe for them due to other medical complications.

- *Procedural Success:* Under the leadership of Dr. Sumanta Mukherjee, the LuX-Valve Plus system (Jenscare Scientific Co., Ltd, China) was successfully and precisely implanted in both patients.

- Immediate Clinical Improvement: Post-procedure evaluations showed optimal valve function and complete cessation of backflow (regurgitation); the patients experienced significant symptom relief without any complications.

*About Dr. Sumanta Mukherjee*

Dr. Sumanta Mukherjee (MBBS, MD, DM, MRCP London), practicing at Apollo Multispecialty Hospitals, Kolkata, is a leading structural heart intervention specialist in India. He has served as a consultant at the renowned St. Bartholomew’s Hospital and Royal Brompton Hospital in London and completed an advanced fellowship in structural heart disease. He has performed over 2,000 TAVI/TAVR procedures and possesses extensive experience in TMVR, TEER (MitraClip/TriClip/Pascal), and TTVR. As a recognized proctor and faculty member, Dr. Mukhopadhyay regularly trains interventional cardiologists across India.

"Tricuspid valve regurgitation has long been one of the most challenging conditions for elderly patients or those with other underlying illnesses. With TTVR, we are pushing the boundaries of what is possible in structural heart interventions. By replacing the valve entirely through a small puncture in the femoral or jugular vein, we can restore patients' quality of life with minimal discomfort and a shorter hospital stay," stated Dr. Sumanta Mukhopadhyay.

A press conference was organized at the Kolkata Press Club to inform the public about this new medical procedure, where Dr. Sumanta Mukhopadhyay explained the details.
